Providers and patients bring different understandings of health and disease to their encounters in the hospital setting. The literature to date only infrequently addresses patient and provider concordance on the reported reason for hospitalization, that is, whether they express this reason in similar ways. An agreement or common ground between such understandings can serve as a basis for future communication regarding an illness and its treatment. We interviewed a convenience sample of patients on the medical wards of an urban academic (...) medical center. We asked subjects to state the reason why their doctors admitted them to the hospital, and then compared their statement with the reason in the medical record. We defined concordance on reported reason for hospitalization as agreement between the patient’s report and the reason abstracted from the chart. We interviewed and abstracted chart data from a total of 46 subjects. Concordance on reported reason for hospitalization was present in 24 (52%) and discordance in 17 (37%); 5 patients (11%) could not give any reason for their hospitalization. Among the 17 patients whose report was discordant with their chart, 12 (71%) reported a different organ system than was recorded in the chart. A significant proportion of medical inpatients could not state their physicians’ reason for admission. In addition, patients who identify a different reason for hospitalization than the chart often give a different organ system altogether. Providers should explore patient understanding of the reason for their hospitalization to facilitate communication and shared decision making. (shrink)

Das Aufgreifen tradierter Motive, speziell des frühneuzeitlichen Vanitas-Topos, lässt sich in zeitgenössischer Literatur, Theater und bildender Kunst beobachten. Insbesondere die Videokunst weist dabei eine Affinität zum malerischen Genre des Stilllebens auf, welches im Kontext des Topos mit moralisch-religiösen und philo- sophischen Fragen verbunden ist und eine Integration unterschiedlicher Zeitmodi ermöglicht. In Anlehnung sowohl an frühneuzeitliche Zeitkonzepte als auch an aktuelle Theorieansätze werden Gestaltungsformen von Zeitlichkeit untersucht, die durch Film- und Videotechnik entstehen, dabei die innerbildlichen Tempora- litäten statischer Stillleben erweitern und (...) die Symbolik der Vanitas resemantisieren, verfremden oder negieren. Es wird nicht nur gezeigt, wie diese Symbole in der Gegenwartskunst eingesetzt werden, um über Vergänglichkeit und Tod zu reflek- tieren, sondern insbesondere, welche Möglichkeiten neue Technologien aufweisen, das Vergehen von Zeit ästhetisch zu gestalten. Transdisciplinary research knits together knowledge from diverse epistemic communities in addressing social-environmental challenges, such as biodiversity loss, climate crises, food insecurity, and public health. This paper reflects on the roles of philosophy of science in transdisciplinary research while focusing on Indigenous and other subaltern forms of knowledge. We offer a critical assessment of demarcationist approaches in philosophy of science and outline a constructive alternative of transdisciplinary philosophy of science. While a demarcationist focus obscures the complex relations between epistemic communities, transdisciplinary (...) philosophy of science provides resources for meeting epistemic and political challenges of collaborative knowledge production. El propósito central de este ensayo es proponer que entre la democracia y el liberalismo existe una relación pragmática. Para ello primero rechazaré la tesis de que este nexo es de tipo lógico-conceptual, así como la idea de que se trata meramente de un nexo contingente, para lo cual retomaré las concepciones de John Stuart Mill y Hans Kelsen. En suma, lo que sostendré es que los modelos democráticos, en tanto métodos de creación normativa, no implican ni las libertades políticas (...) ni las económicas, aunque sí hay lo que llamaré una relación pragmática entre la democracia y el liberalismo, por lo que ser demócrata y antiliberal, o liberal y no demócrata, es una contradicción performativa. El propósito central de este ensayo es mostrar que el debate actual entre el positivismo excluyente y el incluyente se alimenta de, al menos, tres ideas equivocadas acerca de la relación entre el lenguaje jurídico y el moral, la vinculación entre la ciencia y la práctica del derecho y la concepción de lo que es una ciencia normativa. Expuestos estos errores, esbozo una propuesta para entender estas relaciones, fundada en la concepción neokantiana-kelseniana de la ciencia jurídica. De esta doctrina derivo (...) que el lenguaje jurídico y la ciencia del derecho son, respectivamente, el fundamento conceptual del discurso moral y de la práctica jurídica. Asimismo, rechazo la definición que ve en el carácter normativo de la ciencia jurídica la presencia de juicios de valor. En consecuencia, el debate entre el positivismo incluyente y el excluyente debe ser abandonado o, al menos, reformulado. (shrink)

BackgroundThe purpose of this study is to assess the psychometric properties of the French version of the Positivity scale, a self-report measure of positivity, which is the tendency to view and address life and experience with a positive outlook. Positivity is seen as a latent factor underlying multiple cognitive concepts such as self-esteem, life satisfaction, and optimism.MethodsWe recruited 666 volunteers. They completed the P scale online, as well as self-report measures of psychological well-being, self-esteem, satisfaction with life, general health, and (...) personality dimensions. The study sample was randomly split into two sub-groups, one used for exploratory factor analysis and the other for confirmatory factor analysis.ResultsWe found conflictual results related to the uni-dimensionality of the French version of the P scale. We found good internal consistency and high concurrent validity.ConclusionThe French version of the P scale demonstrated good psychometric qualities and is a reliable tool that can now be used by French researchers and clinicians to assess positivity. Sieckmann argumenta que las propias premisas de la teoría pura del derecho desembocan en una disciplina sin objeto de estudio, mientras Kletzer sostiene que el acto constitutivo se da realmente en la aplicación del derecho. De mi defensa se deriva una exégesis de la relación entre filosofía del derecho, ciencia jurídica y derecho positivo conforme (...) al modelo kantiano de la conexión entre la crítica de la razón pura, la ciencia física y los fenómenos naturales. Pour ce projet, il recrute des lettrés par un examen extraordinaire et surveille étroitement leur travail. Seuls deux d’entre eux feront ensuite une carrière de hauts fonctionnaires. Au milieu du XVIIIe siècle, un enseignement de mathématiques est instauré à l’Université impériale. “Actúa de manera tal que los efectos de tu acción sean compatibles con la permanencia de la vida humana genuina en la Tierra”. ¿Cómo podemos entender esta máxima de Jonas? ¿Es demasiado antropocéntrica como para ser interesante para la ética ambiental? ¿Está demasiado limitada a la supervivencia como para tener un significado moral en una ética verdaderamente humana? En primer lugar, podríamos argumentar que no es tan anti-kantiana como para desafiar el “presentismo” actual imperante y nos obliga a tener en (...) cuenta no solo las generaciones futuras, sino también el contexto en el que anticipamos que estas futuras generaciones vivirán. Por lo tanto, deberíamos distinguir dos interpretaciones diferentes de la máxima de Jonas. En una primera etapa, la del desarrollo sostenible, se entendió que considera no solo las necesidades sino también los derechos de las generaciones futuras. En una segunda etapa, la de un Antropoceno y una transición ecológica, significa que dar sentido a la humanidad implica conectar a los seres humanos con la Tierra y otros seres vivos en lugar de oponerse a ellos. Se considera que la ciencia es el espejo de la naturaleza, mientras que el arte imita la vida. De ser así, las representaciones en ambas disciplinas deberían asemejarse a sus objetos. En contra de tales teorías miméticas, argumento que la ejemplificación y no la simple semejanza es crucial. Explico en qué consiste la ejemplificación: una relación referencial de un ejemplar con alguna de sus características. Puesto que la ejemplificación es selectiva, un ejemplar puede diferir de su referente en aspectos no (...) ejemplificados. Ésta es la razón por la cual una idealización, que estrictamente es falsa, puede ofrecer comprensión del fenómeno que le concierne. A partir de analogías con las representaciones pictóricas, muestro cómo un modelo ejemplifica características que comparte con su objeto, pone de relieve la importancia de tales características y con ello proporciona comprensión del sistema en cuestión. Antoinette Fouque cofounded the Mouvement de Libération des Femmes in France in 1968 and spearheaded its celebrated Psychanalyse et Politique, a research group that informed the cultural and intellectual heart of French feminism. Rather than reject Freud's discoveries on the pretext of their phallocentrism, Fouque sought to enrich his thought by more clearly defining the difference between the sexes and affirming the existence of a female libido. By recognizing women's contribution to humanity, Fouque hoped "uterus envy," which she saw as (...) the mainspring of misogyny, could finally give way to gratitude and by associating procreation with women's liberation she advanced the goal of a parity-based society in which men and women could write a new human contract. The essays, lectures, and dialogues in this volume finally allow English-speaking readers to access the breadth of Fouque's creativity and activism. Touching on issues in history and biography, politics and psychoanalysis, Fouque recounts her experiences running the first women's publishing house in Europe; supporting women under threat, such as Aung San Suu Kyi, Taslima Nasrin, and Nawal El Saadaoui; and serving as deputy in the European Parliament. Estoy seguro que la comunidad filosófica de habla española celebrará la publicación de este libro de la Dra. Angélica Rodríguez Ortiz, pues se trata de una argumentación muy bien hilvanada a favor de la idea de que la moral tiene una naturaleza biopragmática. El texto es el resultado de su tesis doctoral, en cuya defensa oral estuve presente como sinodal externo. La principal tesis que la autora defiende, decíamos, es que la moralidad es posible gracias a la naturaleza de nuestra (...) mente, y a que poseemos un lenguaje. En palabras de Wittgenstein, diríamos que el juego del lenguaje moral tiene una base biológica, ya que compartimos una historia natural y un fundamento social, en tanto que la competencia lingüística supone la existencia de una comunidad de hablantes. Dicho de otra manera, nuestra constitución neurofisiológica es uno de los pilares sobre los que descansa la moralidad; el otro es el lenguaje, el cual fija los significados de nuestras palabras, es decir, su uso correcto e incorrecto. El estudio de la prensa periódica temprana en Hispanoamérica enfrenta al investigador con notables obstáculos debidos al tamaño del material y a su estatuto menor como práctica escritural que hizo que no se preservara en forma íntegra y en condiciones idóneas. Aunque falta mucho para que se generalicen, las operaciones de digitalización que llevan a cabo bibliotecas y archivos suponen una transformación radical de este panorama y unas implicaciones difíciles todavía de apreciar. La Segunda Guerra Mundial y las atrocidades cometidas en investigaciones con los prisioneros en los campos de concentración nazis y japoneses, despertaron la conciencia por el desarrollo de los derechos humanos que se habían conquistado paulatinamente a lo largo de la historia. Por ello se conforman una serie de leyes, normas y declaraciones donde se tratan los aspectos bioéticos en los ensayos clínicos. Se debe prestar vital atención a la relación médico-paciente en el curso de las investigaciones y considerar la (...) relación beneficio/riesgo cuando se va a experimentar en seres humanos. La retinopatía diabética proliferativa es una de las afecciones oftalmológicas que más ensayos ha suscitado. El objetivo de este trabajo consiste en analizar algunos aspectos éticos en la experimentación en pacientes con esta afección. The Second World War and the atrocities committed in research with prisoners in Japanese and Nazi concentration camps, awakened consciousness for the development of human rights that had gradually been conquered throughout history. That´s why a series of laws, rules and statements that deal with bioethical aspects in clinical trials, were passed. Special attention must be given to the doctor-patient relationship during the research and consider the benefit/risk ratio when conducting experiments on human beings. Proliferative Diabetic Retinopathy is one of the ophthalmological disorders that has given rise to more trials. The research goal is to analyze some ethical aspects when experimenting on patients with this condition. (shrink)

Ya en su tesis doctoral, Malabou (1996) empieza a elaborar, a partir de una relectura de Hegel, una noción que recorrerá toda su obra: el concepto de “plasticidad”. En este artículo analizamos las principales apariciones de dicho concepto en los primeros trabajos de la autora francesa. En ella, dialoga especialmente, y de manera declarada, con Hegel, Heidegger y Derrida, prometiendo un concepto de la esencia o la forma que incorpore la diferencia sin negarla. Si bien el concepto de “plasticidad” en (...) la obra de Malabou ha sufrido un giro, según el cual ha entrado en un diálogo abierto con la biología y, más concretamente, con las neurociencias contemporáneas, en el presente artículo veremos su estatuto antes de dicho giro. Thus, Manet’s work A bar at the Folies—Bergere contains substantial elements that express and help us to understand both the role of the artist, as the crisis of meaning in the work of modern art and problematic public sphere, treated by Haberma's as (...) the field of social life in which we can develop our public opinion, and that is determined by common sense and rational consensus. In this sense, this article leverages how A bar at the Folies—Bergere questions the social representation of not only the public but the legitimizing institutions that support an artistic truth, to point to an x—ray that presents the structure and the gap between the public and artists, between artistic expression and cultural significance, and thus presents the cultural crisis warned by artists. La religión ha sido la institución más afectada por la pluralización de la realidad social. Esto es debido a que el papel simbólico y global de la religión, en tanto que institución integradora y significativa, queda socavado desde la plausibilidad de sus definiciones sociales de la realidad. La causa de esta situación se encuentra en que las personas viven -conciencia subjetiva- nuevos roles institucionales, nuevos esquemas interpretativos, nuevos valores y creencias. Sin embargo, si algo permanece invariablemente constante en la vida (...) diaria de las personas es, afirma Berger, el temor a las situaciones marginales y a la anomia. Ni el proceso de secularización con sus universos simbólicos (filosofía diurna y teodiceas seculares), ni el resurgimiento religioso y global (desecularización público-política) han podido dar una explicación plausible y convincente de estos fenómenos que tanto afectan a la conciencia y a las identidades subjetivas de los individuos. En este sentido, y tras el fracaso de las anteriores cosmovisiones en su intento de dotar de sentido a la realidad y la vida ordinaria de las personas, Berger recurre a una nueva instauración del «nomos religioso» Religion has been the institution most affected by the plurarization of social reality. This is because the overall symbolic role of religion as an institution inclusive and meaningful, is undermined from the plausibility of their social definitions of reality. The cause of this situation is that people live-subjetive consciousness-new institutional roles, new interpretative schemes, new values and beliefs. If something always remains constant in the daily lives of the people, says Berger, the fear of marginal situations and anomie. Neither the process of secularization with its symbolic universe (diurnal philosophy and secular theodicies) and the religious revival and global (desecularization public-politic) could give a plausible and convincing explication of these phenomena that affect the subjective consciousness and identity of individuals. In this regard, and after the failure of previous worldviews in their attempt to give meaning to reality and the ordinary life of people, Berger uses a new establishment of «nomos religioso». El uso del espacio virtual invita a una reflexión ética sobre su impacto moral. Sociólogos, psicólogos y filósofos son incapaces acordar sus implicaciones éticas. El presente artículo examina el espacio virtual a través de la teoría de La Construcción Social de la Realidad de Peter L. Berger y Thomas Luckmann, que sostiene que el otro significante conforma el carácter del individuo. Se argumenta que el ciberespacio produce un progreso moral. Por un lado, la multiplicidad de otros significantes representa un (...) obstáculo en la socialización del individuo. Por otro y más importante, los otros significantes constituyen nuevas verdades que favorecen un nuevo orden moral. Este resultado se explica mediante el concepto de otro generalizado originario del filósofo George H. Mead. En conclusión, se propone que el espacio virtual conforma el carácter moral del individuo de forma positiva. (shrink)

: The shepherd embodies a strange and disturbing society. Isolated, marginal, it forms a world apart and evolves in a wild space where mountains, valleys, meadows or forests make up the framework of its activity. In this non-domesticated nature the human presence is suspect. This confusing being is very often represented with an animalized, almost monstrous or deformed body which becomes a metaphor for social order. This grotesque body translates the prejudices of urbanites and elites. It fuels sexual fantasies and (...) symbolizes the disturbances of the natural and social order: freed from the laws of the city, it can try to seduce the young ladies and be insolent in front of the knights. It is a threat to the established order. But the shepherd also carries virtues. It is a model of Christian life: humility and holiness. It belongs to an original humanity, the form closest to divine creation. Two styles of representation of the shepherd therefore oppose the extremes. From texts from the end of the Middle Ages, we will see how the shepherd is an imaginary in opposition to an organized society but that his image has evolved positively until becoming the guardian of traditions and a being who lives in harmony with nature. Isolé, marginal, il forme un monde à part et évolue dans un espace sauvage où les montagnes, vallées, prés ou forêts composent le cadre de son activité. Dans cette nature non domestiquée la présence humaine est suspecte. Cet être déroutant est bien souvent représenté avec un corps animalisé, quasi monstrueux ou déformé qui devient une métaphore de l’ordre social. Ce corps grotesque traduit les préjugés des urbains et des élites. Il alimente les fantasmes sexuels et symbolise les dérèglements de l’ordre naturel et social : affranchi des lois de la cité, il peut tenter de séduire les demoiselles et se montrer insolent face aux chevaliers. C’est une menace contre l’ordre établi. Mais le berger est également porteur de vertus. C’est un modèle de vie chrétienne : humilité et sainteté. Il appartient à une humanité originelle, forme la plus proche de la création divine. Deux styles de représentation du berger opposent donc les extrêmes. A partir de textes de la fin du Moyen Age, nous verrons comment le berger est un imaginaire en opposition avec une société organisée mais que son image a évolué positivement jusqu’à devenir le gardien des traditions et un être qui vit en harmonie avec la nature. Este ser confuso a menudo se representa con un cuerpo animalizado, casi monstruoso o deformado que se convierte en una metáfora del orden social. Este cuerpo grotesco traduce los prejuicios de los urbanitas y las élites. Alimenta las fantasías sexuales y simboliza las perturbaciones del orden natural y social: liberado de las leyes de la ciudad, puede tratar de seducir a las jóvenes y ser insolente frente a los caballeros. Es una amenaza para el orden establecido. Pero el pastor también posee virtudes. Es un modelo de vida cristiana, de humildad y santidad. Pertenece a una humanidad original, la forma más cercana a la creación divina. Dos estilos de representación del pastor, por lo tanto, se oponen. A partir de los textos del final de la Edad Media, analizamos la imagen del pastor en oposición a toda sociedad organizada y la evolución positiva de su figura hasta convertirse en guardián de las tradiciones y en un ser que vive en armonía con la naturaleza. Truth is standardly considered a requirement on epistemic acceptability. But science and philosophy deploy models, idealizations and thought experiments that prescind from truth to achieve other cognitive ends. I argue that such felicitous falsehoods function as cognitively useful fictions. They are cognitively useful because they exemplify and afford epistemic access to features they share with the relevant facts. They are falsehoods in that they diverge from the facts. Nonetheless, they are true enough to serve their epistemic purposes. Theories that contain (...) them have testable consequences, hence are factually defeasible. (shrink)

According to Rosenthal's higher-order thought (HOT) theory of consciousness, one is in a conscious mental state if and only if one is aware of oneself as being in that state via a suitable HOT. Several critics have argued that the possibility of so-called targetless HOTs—that is, HOTs that represent one as being in a state that does not exist—undermines the theory. Recently, Wilberg (2010) has argued that HOT theory can offer a straightforward account of such cases: since consciousness is a (...) property of mental state tokens, and since there are no states to exhibit consciousness, one is not in conscious states in virtue of targetless HOTs. In this paper, I argue that Wilberg's account is problematic and that Rosenthal's version of HOT theory, according to which a suitable HOT is both necessary and sufficient for consciousness, is to be preferred to Wilberg's account. I then argue that Rosenthal's account can comfortably accommodate targetless HOTs because consciousness is best understood as a property of individuals, not a property of states. (shrink)

Hope is often seen as at once valuable and dangerous: it can fuel our motivation in the face of challenges, but can also distract us from reality and lead us to irrationality. How can we learn to “hope well,” and what does “hoping well” involve? Contemporary philosophers disagree on such normative questions about hope and also on how to define hope as a mental state. This article explores recent philosophical debates surrounding the concept of hope and the norms governing hope. (...) St Catherine of Siena, Doctor of the Church, was one of the most illustrious figures of the late Middle Age. Her abundant correspondence cons-titutes a first-hand tes-timony on the Christian life of that epoch. A good number of her letters is addres-sed to women, and reveal a deep and demanding spiritual outlook on the role of the Christian woman -an outlook that values the characteristics proper to the female condition-.

I defend the thesis that friendship can constitutively require epistemic irrationality against a recent, forceful challenge, raised by proponents of moral and pragmatic encroachment. Defenders of the “encroachment strategy” argue that exemplary friends who are especially slow to believe that their friends have acted wrongly are simply sensitive to the high prudential or moral costs of falsely believing in their friends’ guilt. Drawing on psychological work on epistemic motivation (and in particular on the notion of “need for closure”), I propose (...) a different picture of what friendship requires in the doxastic realm. I argue that contrary to what the encroachment strategy suggests, exemplary friends’ belief formation ought not be guided by a concern with accuracy or error avoidance, but instead by a need to avoid a “specific closure” – namely, a need to avoid concluding in their friends’ guilt. I propose that exemplary friendship often generates a defeasible, doxastic obligation to exemplify such a need, despite its inherent corrupting effects on exemplary friends’ epistemic faculties.
